What about this box?

domain guys -> rule: (square root of the input) + 4 -> range guys
 

The official notation would be

f( x ) = (square root of x) + 4 ... x guys go in ... and spits "(square root of x) + 4" out

 

So,
 		 f( 9 ) = (square root of 9) + 4 = 3 + 4 = 7
  like letting x = 9
